Is it possible to add a form template to a notebook after it has been created?

Yes. Just go to the Notebooks tab, click the gear icon on the Notebook, select Edit, then go to Form Templates tab and add the form template.   When the mobile app syncs, the new form will be available on the mobile users’ devices.
